COVID-19 VACCINES

Oakhill Clinic will be participating with the roll out of the government funded COVID-19 Vaccines 1b phase.

We will be running a special COVID vaccine clinic.

This clinic will ONLY run on Mondays

You MUST make a special appointment JUST for the COVID vaccine.

You will not be given the vaccine at your standard appointment time.

Please note that we are trying our very best to have all of our patients vaccinated as soon as possible. We will be given a certain number of doses per week from the government. We cannot control how many we get, so we please ask that you be patient with us.

We will be making appointments from the 22nd of March.

FOR NEW PATIENTS ONLY

For those who are not a patient at the clinic please download the consent form, fill it in and email it back to us. Once we have received your email we will register your details and will call you to organize your appointment.

Younger adults with an underlying medical condition, including those with a disability and critical high risk workers including defence, police, fire, emergency services and meat processing MUST provide proof.

New patients only: Please bring the following to the appointment ;

  • Medicare card (If you do not have a medicare card we can not see you until you have registered for a Individual Health Identification (IHI) please refer to Services Australia)
  • Proof of medical condition, preferably a health summary from your GP.
  • Proof of Employment
  • Face mask (compulsory at our clinic).
  • Ideally wear a short sleeve garment.
